Notice of Cybersecurity Incident


Dental Safari Company is committed to protecting the privacy and security of the information in
our care. Regrettably, we experienced a cybersecurity incident that involved some of that
information. This notice explains the incident and measures we have taken in response.

On March 14, 2023, we experienced a ransomware incident that affected our internal computer
systems and on-premises file server. That same day, we took our systems offline and
permanently decommissioned the affected file server. The impact to those systems was such that
a limited amount of clinical records, including exam and cleaning records for patients seen in
Indiana and Illinois prior to March 14, 2023, were not recoverable.

We are currently analyzing the files known to have been on the file server to identify the
information they contained. This analysis is ongoing, and we will send letters to individuals whose
information may have been on the file server once that investigation is complete. We will also
update this notice as we obtain more information.

At this time, we have no indication that any patient information has been accessed or misused.
However, out of an abundance of caution, we have established a dedicated call center for
individuals with questions, which can be reached at 888-596-5146, Monday through Friday,
between 9:00 a.m. and 9:00 p.m. EST.

As always, we recommend that our patients review all statements received from health care
providers and contact the provider immediately if a statement contains services that were not
received. We sincerely regret any inconvenience this incident may cause our patients and their
families. To help prevent something like this from happening in the future, we have moved to
cloud-based systems and strengthened our security protocols to further safeguard the
information in our care.
